Case Manager RN | $10K Sign-On Bonus

Job Summary

We are seeking an experienced Case Manager Registered Nurse (RN) to join a growing acute care hospital team. This is a full-time permanent opportunity offering competitive pay, excellent benefits, and career growth.

Schedule

• Day Shift: 8:00 AM – 4:30 PM
• One weekend every 4 weeks

Compensation & Benefits

• Pay Range: $31.94 – $43.92/hr (based on experience)
• Sign-On Bonus: Up to $10,000
• Shift Differentials:
• • Evenings: +$2.50/hr
  • Weekends: +$2.00/hr

Responsibilities

• Perform patient assessments and develop individualized care plans
• Coordinate discharge planning and post-hospital services
• Review medical necessity, admission status, and resource utilization
• Collaborate with physicians, nurses, and interdisciplinary teams
• Conduct utilization reviews and communicate with insurance providers
• Identify barriers to care and facilitate quality patient outcomes
• Educate patients and families on available healthcare resources
• Promote efficient patient throughput and continuity of care

Requirements

• Active RN License (Compact License Accepted)
• ADN or Nursing Diploma Required
• BSN Preferred
• Minimum 3 years of recent Case Management experience in an acute care setting
