Registered Nurse – Case Manager

Duration: 13 Weeks
Location: 4101 Torrance Blvd, Torrance, CA 90503
Start Date: ASAP
Schedule: Days | 5×8s | Monday–Friday | 40 Hours/Week

Position Overview

We are seeking an experienced Registered Nurse Case Manager with a minimum of 2 years of recent acute care case management/discharge planning experience. The RN Case Manager will coordinate patient care, facilitate safe and timely discharge planning, assess patient needs, and collaborate with multidisciplinary teams to support appropriate utilization of healthcare resources.

Job Requirements

  • Minimum Experience: 2 years of recent acute care Case Management/Discharge Planning experience

  • Travel Experience: Required

  • First-Time Travelers: Accepted; must complete screening

  • Patient Ratio: Approximately 1:20

  • Floating: No

  • Weekend Requirement: Every other weekend

  • Holiday Requirement: Must be available to work holidays

  • RTO: No more than 7 days

  • California RN License: Required; pending license accepted if fully licensed by start date

  • EMR: Epic – Required

  • Certifications: None specifically required

Core Responsibilities

  • Perform comprehensive patient assessments and identify case management and discharge planning needs.

  • Develop and coordinate individualized plans of care in collaboration with patients, families, physicians, nurses, and other healthcare professionals.

  • Facilitate timely and appropriate discharge planning across the continuum of care.

  • Coordinate transitions of care, including referrals to home health, hospice, LTAC, rehabilitation, skilled nursing, and other post-acute services.

  • Evaluate medical necessity and appropriate level of care using evidence-based guidelines and established criteria.

  • Coordinate insurance benefits, prior authorizations, and other payer-related requirements as applicable.

  • Identify barriers to discharge and develop strategies to address clinical, social, financial, and logistical needs.

  • Coordinate DME and other services required for safe discharge.

  • Communicate with insurance companies, providers, patients, and multidisciplinary teams to facilitate appropriate care.

  • Monitor patient progress and revise the plan of care based on changing clinical and discharge needs.

  • Support utilization review activities, including admission criteria and medical necessity review.

  • Assist with appeals and denials processes when required.

  • Maintain accurate and timely documentation in Epic.

  • Ensure compliance with CMS, HIPAA, DRG, and applicable regulatory requirements.
