Weekend REQ: typically one full weekend per 4 weeks, subject to dept. need
RTO Restrictions: No more than 7 days

Special Procedures/Unit Details:
•    Experience with HMOs, IPAs, etc.
•    Experience with Assessing patients, Care Coordination, and Discharge Planning
•    Computerized documentation, required.
•    2-year acute medical Care Manager experience performing Care Coordination/Discharge planning
