Position Summary
Randstad is seeking a Case Manager to join the Center for Urban Community Services (CUCS) at Williamsbridge Gardens in the Bronx, serving formerly homeless families and singles with mental illness or special needs. This role delivers holistic, strengths-based support to a dedicated caseload through direct outreach, crisis intervention, benefit coordination, and housing retention services. Working both on-site and in the community, the Case Manager helps clients build self-sufficiency and maintain permanent housing stability.
Key Responsibilities
Case Management & Engagement: Develop individualized service plans (ILPs); provide regular outreach, medication self-administration support per protocols, and community escorts to medical, legal, and benefit appointments.
Benefits & Housing Retention: Assist clients with securing public benefits (SSI/SSDI, Medicaid, SNAP), rep payee funds, move-in orientation, recertifications, inspection prep, landlord relations, and daily living skills (ADLs) including hygiene, cleaning, and pest remediation.
Crisis Response & Operations: Deliver trauma-informed crisis intervention, coordinate with emergency services, manage PackTite bed bug remediation protocols, facilitate support/recreational groups, and complete timely EHR progress notes to meet monthly contact targets.
Qualifications & Requirements
Education: High School Diploma or GED required (Bachelor’s degree in Social Work, Psychology, or related field preferred).
Experience: No formal experience required (prior background in human services or working with vulnerable populations preferred).
Core Competencies: Crisis de-escalation skills, communication, ability to conduct home/community visits, and dedication to recovery-oriented care.
