NOW HIRING: CASE MANAGER – COMMUNITY GARDEN PROGRAM

Location: New York City, NY
Pay: $24–$26/hour
Schedule: Monday–Friday | 9:00 AM–5:00 PM
Bilingual Spanish Required

Are you a compassionate and motivated human services professional who enjoys working directly with individuals in the community? We are seeking a dedicated Case Manager to join a Community Garden Program in NYC!

POSITION RESPONSIBILITIES:

Conduct initial client assessments
Complete monthly in-home client assessments
Develop and implement individualized service plans
Provide a full range of social services and ongoing case management support
Travel to clients’ homes throughout NYC using mass transit
Maintain accurate and timely case documentation
Collaborate with clients, families, community resources, and program staff
Connect clients with appropriate services and resources

QUALIFICATIONS:

Bachelor’s degree in Social Work, Human Services, Sociology, or a related field
Bilingual Spanish – Required
Comfortable traveling throughout NYC using public transportation
Strong communication, assessment, and organizational skills
Ability to work independently and effectively manage a caseload
Compassionate, dependable, and community-focused

PAY: $24–$26 PER HOUR
