Case Manager MSW

As a Case Manager MSW at Novant Health Ballantyne Medical Center and Novant Health Mint Hill Medical Center, you will serve as a leader of the patient assessment and complex discharge planning process.  You will float between two of our community hospitals based on census need, allowing you to build relationships throughout both hospitals.  Mint Hill Medical Center is a 46 bed facility recently voted in the top 10 community hospitals in the nation.  With 36 inpatient beds, Ballantyne Medical Center is our newest facility in the Charlotte area. 

This part-time weekender position offers you the perk of working 24 hours each week and earning 8 hours of incentive pay.  Part-time benefits are available.  You must be able to orient for a minimum of 30 hours per week for 4-8 weeks, requiring some weekday hours during this time.

• Education: Master's Degree in Social Work from school accredited by the Council on Social Work Education, required.
• Experience: Two or more years professional social work experience or two or more years healthcare experience, strongly preferred.

• Serves as a leader of the complex discharge planning process and a valued member of the healthcare team
• Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans
• Collaborate with RN Case Managers for guidance with medically complex cases
• Serve as a resource for discharge planning and care coordination for psychosocially complex cases
• Responsible for facilitating coordination of care
• Maintain current knowledge of available community resources and post acute care options
• Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health
