We are seeking a skilled professional to coordinate patient care during acute care hospitalization and manage all aspects of the discharge planning and transition process.

Duties

• Coordinate patients through the acute care hospitalization process.
• Manage all aspects of discharge planning and transition processes.
• Perform utilization management and quality screenings.
• In-Patient Case Management is required.

Skills

• 2 years of acute care experience strongly preferred.
• Experience in Case Management and Managed Care.
• In-Patient Case Management is required.
• Experience with Sub-acute patients is a must.

Schedule Notes

• 8 AM - 4 PM
• Rotating weekends every 4th or 5th week.
• 1 summer and 1 winter holiday required.
