Salary Range: $42,900/year ($22.00/hour) - $54,600/year ($28.00/hour)
The above salary range represents Options for Community Living's good faith and reasonable estimate of potential compensation that may be offered to a successful applicant for this position at the time of this job posting and may be modified in the future. Schedule Options: (37.5 hours/week)
Location: In person based out of our Ronkonkoma office, with field visits required.

Responsibilities:
The Residential Support Case Manager provides supportive case management services to tenants with mental illness in supportive housing and provides on-site visits to scattered site locations. The Case Manager will work with the tenant, treatment team, and the Mobile Residential Support Team (MRST) to identify issues and barriers that may prevent the individual from maintaining stability in the community. Through a mutually agreed goal plan, the MRST Case Manager will assist the tenant in seeking solutions to achieve mental health and health care goals, sobriety, housing stability and to prevent where possible, emergency room usage and inpatient hospitalization.
