SummaryUnder the direction of the Pathway Home Program Supervisor and Program Manager, the On-Site Case Manageris responsible for screening, on-site case management, eligibility, service coordination, and triage for InterimHousing placement and transition care to permanent housing case management Pathway Home participants.Essential Functions On-Site Case Manager is responsible for a caseload of Interim Housing participants. Provides Case Management, linkage, and referrals with coordination with the supportive services team,and onsite monitors. Monitors direct progress and follows up with participants while in the interim housing sites. Works collaboratively with the internal resources, DHS, and partner agencies in service delivery. Participates in case conferences/reviews with providers, supportive services team, DHS, and LAHSA. Provides individual and group training sessions. Conducts life skills training with participants, and support groups. Document client information including writing progress notes in clarity. Provides crisis intervention as needed. Conduct room checks daily at all interim housing sites. Helps track program client demographics and treatment progress. Enter data into CLARITY/HMIS within 24 hours. Prepare project reports in accordance with funding requirements. S/he will assist in training and provide support to necessary staff, providers, and other project partnersparticipating in the project. The on-site case manager will participate in all mandatory program anddivision meetings and trainings, as assigned by his/her supervisor and division director. Prepare reports in accordance with program requirements and Division policies. Administer and screen all cases using the VI-SPDAT / Assessment tool. Enforces program rules and regulations. Coordinate with other projects staff and partners to provide housing placement services and supportiveservices to facilitate clients successful transition to permanent housing and coordinate services, to fasterhousing stability. Work with the Housing Locator Teams to locate and secure housing. Assist clients to apply for units and attend landlord interviews while in coordination with the supportiveservice team. Collaborate effectively with all SPA 6 CES, LAHSA staff and landlords. Work with property management staff to address issues as they arise. Assess client satisfaction on a continual basis. Work with the client to complete an individualized housing stability plan, and budget to ensure householdcan sustain housing after their subsidy has ended. Verify program eligibility documents prior to program enrollment and at the time of the financialassistance request submission. Collaborate and coordinate with DMH, DPSS, LAUSD, Work Source, and Sub- Providers. S/he will assist clients with identifying the most appropriate housing intervention. Coordinate with other staff and collaborative partners to provide ancillary services to achieve housingstability; follow-up with clients weekly, at minimum; maintain interagency consultation, coordination,and referrals as it relates to housing. Maintain up-to-date and accurate documentation in client’s files for service coordination/casemanagement, housing placement, rental assistance, supportive referral assistance. Some evenings and weekends are required. Prepare project reports in accordance with funding requirements. The On-site Case Manager will participate in all mandatory program and division meetings and trainings,as assigned by his/her supervisor and Division Director or Deputy Director. Prepare reports in accordance with program requirements and Division Policies. Conduct criminal background checks on all pre-eligible applicants and household members over 13 yearsof age. Maintain appropriate boundaries: and adhere to SSG’s Code of Ethics and HOPICS’ Core Values. Represent the agency in a professional manner at meetings and community events. Maintain weekly reports as instructed by supervisor/manager. Regular attendance required. Other duties may apply.Secondary FunctionsPerform other duties as assigned by the Program Supervisor / Program Manager, Associate Director, and orDeputy Director. Answer phones and route incoming calls; serve as back-up to the Intake Coordinator in his/herabsence.Minimum Qualifications - Knowledge, Skills and Abilities RequiredAssociate degree in social services or other administrative, business or housing field is required from anaccredited or state approved college or university, with a minimum two (1) year job related experience workingwith homeless individuals and families OR 1 to 2 years’ experience working in social service field; casemanagement and homeless program experience preferred. If in recovery, a minimum of three (3) years of beingdrug and alcohol free is required. Working knowledge of Microsoft Word, Excel, and other database programs.Knowledge of resource development, case management and documentation. Ability to work with clients fromdiverse cultural, ethnic, and socio-economic backgrounds. Ability to communicate effectively, both written andorally. Verification of Employment Eligibility and Background Clearance. TB test required annually or as needed,CPR and First Aid Certification required every two years or as needed with company and valid Driver’s Licenseand auto insurance required. Reliable transportation is required. Ability to communicate effectively, both written and orally.Non-Essential Qualifications:Experience using HMIS and at least six months to one year of experience preferred.Supervisory Responsibilities:This position does not have any supervisory responsibilities.Environmental Conditions (Working Conditions)This position is responsible for working in “client friendly” environments and is required to visit other sheltersand homeless access centers as part of their duties. Local automobile travel is required. There is someresponsibility to work in noisy environments where children and adults are present.Physical RequirementsThe On-Site Case Manager typically spends time sitting, standing, typing, thinking, writing, walking, driving,carrying (max. 25 lbs.), listening, speaking.Mental RequirementsThis position will require the individual to be able to handle any/all the following: constant distractions,interruptions, uncontrollable changes in priorities/work schedules: be able to process information, think andconceptualize.
