Case Manager / Life Skills Coordinator – Residential

Location: New York, NY
Job Type: Contract to Hire

Mitchell Martin Healthcare is partnering with a leading nonprofit human services organization to hire a Case Manager / Life Skills Coordinator for a residential program in New York City.

Schedule

Standard shifts include one of the following schedules:

  • 8:00 AM – 4:00 PM
  • 8:30 AM – 4:30 PM
  • 9:00 AM – 5:00 PM

Additional schedule requirements:

  • Staff are required to work one late day per week, generally starting one hour later than their regular schedule.
  • Saturday coverage is required approximately once per month.
  • Employees who work a Saturday will receive a weekday comp day during the same week.
  • After completing one month in the position, employees become eligible for one hybrid workday per week, based on program needs and approval.

Responsibilities

  • Manage an assigned caseload and meet with clients regularly to assess needs and provide ongoing support.
  • Develop and maintain individualized service plans and client documentation.
  • Connect clients with healthcare, behavioral health, housing, social service, and community resources.
  • Coordinate and accompany clients to medical, psychiatric, and other appointments as needed.
  • Advocate for clients and help them navigate healthcare and social service systems.
  • Provide education and coaching related to independent living and daily life skills.
  • Assist clients with self-care, budgeting, money management, housing, social skills, and community integration.
  • Use your creativity to plan and lead engaging group activities, recreational programs, community outings, and trips for residents.
  • Identify new activities, events, and community resources that align with residents' interests and goals.
  • Facilitate groups focused on life skills, wellness, personal development, and independent living.
  • Assess clients' strengths, needs, and areas where additional support is required.
  • Provide crisis intervention and respond appropriately to urgent client needs.
  • Maintain accurate and timely case notes, assessments, service plans, and other required documentation.
  • Research community resources and make appropriate referrals.
  • Collaborate with internal staff, healthcare providers, community organizations, and other service providers.
  • Maintain client confidentiality and comply with HIPAA requirements.
  • Participate in training and professional development.

Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, Sociology, Counseling, or a related field.
  • 2+ years of experience in case management, human services, social services, residential services, supportive housing, behavioral health, or a related setting.
  • Experience working with individuals experiencing homelessness, mental health challenges, substance use, HIV/AIDS, or other complex social-service needs is strongly preferred.
  • Knowledge of community resources, benefits/entitlements, healthcare systems, and social services.
  • Strong communication, organization, and interpersonal skills.
  • Ability to build positive and professional relationships with clients.
  • Strong documentation and time-management skills.
  • Crisis intervention and problem-solving abilities.
  • Proficiency with Microsoft Word, email, internet-based systems, and electronic records.
  • Knowledge of HIPAA and confidentiality requirements.
