This is a full-time, benefits-eligible position through June 30, 2027, with potential opportunities for transfer or continuing employment based on operational needs and performance.

What You Will Do to Change Lives

Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Case Manager III is distinguished from Case Manager II by the additional education, years of experience, and ability to proficiently perform the job functions and meet position standards.

Schedule: Full-Time | 8:00 AM - 4:30 PM | Monday - Friday

Compensation: Expected starting wage range is $23.00 - $26.25.

Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

What You Bring to the Table (Must Have)

• Bachelor's degree in Social Services or Human Services and (1) year of experience or High School Graduation or G.E.D. equivalent and four (4) years of direct services in Mental Health or community services; an Associates degree may be substituted for two (2) year of experience.
• Valid and current driver's license, and personal vehicle insurance with your name listed as a driver.
• Willingness to use your personal vehicle to drive clients to appointments and groups, etc.
• Willingness to use your personal vehicle to attend meetings, etc.

Service participation is voluntary and not a requirement of residing at The Shores and Pacific Village. Telecare works with tenants to engage them in services according to their behavioral health and case management needs. Services are provided throughout the community based on preference: on-site in The Shores and Pacific Village PSH community space; in individual units; in the supportive service office; or in the community, if appropriate. These services focus on housing stability and retention.
