What You Will Do to Change Lives
Under direct supervision, serves as a primary support and case manager for individuals with mental illness and other co-occurring disorders by assisting them to live as independently as possible in their chosen community. Case Manager III is distinguished from Case Manager II by the additional education, years of experience, and ability to proficiently perform the job functions and meet position standards.

Full-Time | AM | Shifts: 8:30 AM - 5:00 PM | Days: Monday - Friday
Expected starting wage range is $21.49 - $26.25. Telecare applies geographic differentials to its pay ranges. The pay range assigned to this role will be based on the geographic location from which the role is performed. Starting pay is commensurate with relevant experience above the minimum requirements.

About the Program: Home First North FSP is Orange County's Housing Full Service Partnership (FSP) program focused on providing housing-supportive services and behavioral health services to individuals residing in Orange County. Our purpose is to support members in maintaining stable housing, including resolving housing challenges and providing case management and behavioral health-focused services. Our staff creates a supportive environment, including services that are based on individual needs, hopes, and dreams. Home First North FSP members have access to a multidisciplinary team that includes peer specialists, case managers, a psychiatrist, a nurse practitioner, masters-level team leads, and clinicians who have experience in providing housing-supportive services and behavioral health-focused services. Program serves: 1) Residents of Orange County, ages 18 and older, who have been diagnosed with a severe mental illness and 2) Individuals who have been housed and are currently at risk of losing housing in Orange County.
