JOB TITLE Case Manager II Virginia - Intellectual & Developmental Disabilities
SUMMARY:
Coordinating services for individuals with developmental disabilities (DD) within a caseload. This includes the administrative maintenance of all case records and any corresponding case management duties assigned to them.
EDUCATION AND EXPERIENCE:
Education: Bachelor's degree in behavioral health related area preferred.
Licensure: Must possess or obtain highest level of licensure appropriate to degree. Valid drivers license.
Certification: Must possess or obtain highest level of certification appropriate to degree.
Experience: Experience in mental health and/or intellectual disabilities preferred.
Knowledge/Skills: Verbal/written communication skills.
Ability to maintain composure in difficult situations.
Ability to safely operate motor vehicle.
EQUIPMENT:
Computer, fax, copier, motor vehicle and any other equipment required to perform the functions of the position.
MAJOR DUTIES AND RESPONSIBILITIES:
The process of service coordination requires that the case manager possess the knowledge, skills, and abilities necessary to perform the following activities:
Assessing - evaluating and determining all of the service needs of the individual and/or family. This includes completing a VIDES assessment (to determine qualification for waiver services) and participation and coordination of Supports Intensity Scale (SIS) assessments.
Planning - formulating written Individual Service Plans. These Individual Service Plans must be person-centered and should address the following:
strengths and weaknesses of the individual
service needs that clearly identify specific goals and objectives. These goals and objectives should clearly as possible identify specific target dates for the attainment of these services or the resolution of needs.
responsibility for the coordination and the delivery of identified services.
development and discussion of employment goals
Linking - Advocating for the individual in contacting and actively facilitating constructive relationships with service resource agencies, programs, individuals, etc. This includes linking individuals to REACH crisis services and managed care organizations.
Referral - Referral of individual to other community agencies, medical providers, Regional Support Teams, etc. to improve overall health of individual
Monitoring - An ongoing process of reviewing and updating Individual Service Plans, insuring that services are intact, and that they are adequately meeting the needs of the individuals. Includes up-to-date and accurate record-keeping (progress notes, quarterly reviews and other required documentation) in WAMS, CMCSB's electronic health record, etc.
