Expectations
As a Case Manager, you'll:
- Own a caseload and guide clients through assessment, service planning, and referrals — always centering their voice and choice in the process
- Connect people to what they need — substance abuse and mental health treatment, transportation, and coordinated care with partner providers
- Show up in crisis moments — conducting risk assessments, coordinating crisis plans with psychiatric providers, and linking clients to hospitals and crisis teams when it matters most
- Keep meticulous, compliant records that protect your clients and strengthen our programs
- Drive shared efforts, meeting people where they are — sometimes literally, behind the wheel, getting them to the appointments that change their trajectory
What You Bring
- A high school diploma or GED (bonus points for an Associate's or Bachelor's degree)
- Willingness to learn and train but 1–2 years of case management or supportive services experience with homeless or underserved populations is a plus
- A working knowledge of homelessness, mental health, and substance abuse issues
- Sharp time-management skills
- Communication skills built for conflict resolution
- Integrity, patience, and a genuine commitment to the mission
- Comfort with ambiguity and a fast-moving environment
- A valid driver's license and a clean traffic record

Schedule: General business hours, Monday–Friday, with some evenings, weekends, as needed.
