Monday - Friday, 12:00PM - 8:00PM OR Tuesday - Friday 12:00PM - 8:00PM and Saturday 9:00AM - 5:00PM

Job Summary

Pilsen Wellness Center is seeking a dedicated and compassionate Case Manager to participate in the implementation of the individual's service plan, specifically designed to increase an individual's skills in independence in daily living, economic self-sufficiency and community integration.

Responsibilities

• Assists the client in achieving independence (e.g., housing, finances)
• Assists with supported employment/education as needed
• Maintain timely and accurate clinical documentation
• Helps participants to access the services specified in their service plans
• Adhere to ethical and confidentiality standards

Qualifications

• High School Diploma or GED
• Active Drivers License
• Bilingual (English/Spanish) preferred
• 3-5 years of administrative and clerical assistance experience
• Cultural humility and the ability to build trust with diverse populations
• Strong documentation and collaboration skills
