JOB SUMMARY
The Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. The Colorado Rapid Re-Housing for Re-Entry program serves unhoused justice involved individuals and utilizes a housing-first model to assist participants in obtaining stable permanent housing. Job duties include conducting needs-based and program eligibility assessments, engaging in community outreach, providing case management services, and program-wide service coordination. Case Manager 2 may be charged with developing program-specific specializations
relevant to the needs of participating households, and is expected to be aware of, and consistently apply,
best practice standards in their applicable field of work. Further, Case Manager 2 may be charged with
serving as a mentor to junior personnel to support the development of knowledge and experience in best
practice standards throughout their work group

Full-Time or Part-Time as indicated in posting.
LOCATION · Officed out of Pueblo location. Travel throughout program service area. VSS Field Offices located in: Denver, Greeley, Fort Collins, Grand Junction, Durango, Alamosa, Pueblo, and Colorado Springs.
