Case Manager / Client Care Coordinator for Home Care Agency

Care Quality Assurance: Perform routine field supervisory visits, evaluate caregiver performance, and ensure care plans are being followed accurately. Key Responsibilities:Assessments & Onboarding: Conduct in-person client assessments, complete home safety checks, and design personalized care plans for new senior clients.

We are seeking a detail-oriented, compassionate Part-Time Case Manager to handle client intake assessments, care plan development, and field quality assurance 3 days per week.Key Responsibilities:Assessments & Onboarding: Conduct in-person client assessments, complete home safety checks, and design personalized care plans for new senior clients.Care Quality Assurance: Perform routine field supervisory visits, evaluate caregiver performance, and ensure care plans are being followed accurately.Client & Family Communication: Serve as a trusted point of contact for family members, addressing care plan adjustments and client feedback.Caregiver Orientation: Brief caregivers on specific client needs and condition updates prior to assignment start dates.Qualifications:2+ years of case management, intake, or social work experience in home care, senior living, home health, or Regional Center caregiving.Hands-on knowledge of senior care needs, ADL support, and dementia care protocols.Excellent communication skills and reliable personal transportation for local East Bay travel.
