Position Overview: TASC is seeking a compassionate and motivated Case Manager to support clients involved in the criminal legal system who are struggling with substance use and behavioral health challenges. Case Managers in our Adult Criminal Justice and Treatment Services (ACJS) division work closely with individuals to provide assessments, referrals, and case management services that support recovery and long-term stability
Key Responsibilities:
Conduct comprehensive assessments and develop service plans tailored to client needs
Facilitate intake and enrollment into TASC programs
Provide referrals and crisis intervention for behavioral health, housing, employment, and other supports
Collaborate with community partners and internal staff to ensure coordinated care
Apply DSM-5 and ASAM criteria in evaluating client needs and treatment referrals
Maintain timely and accurate documentation, including progress notes, reports, and discharge summaries
Monitor progress toward service goals and ensure compliance with all reporting systems
Participate in staff meetings, trainings, and multi-disciplinary team discussions
Support clients in navigating systems and accessing essential services
Ideal Candidate Profile:
Education: Bachelor’s degree preferred, or at least three years of relevant professional or lived experience
Certification: Willingness to obtain CADC or CARS certification within two years
Skills: Excellent communication, time management, and documentation skills
Technology: Proficiency in Microsoft Office suite and ability to work in database systems
Values: Commitment to diversity, equity, and inclusion; creative and flexible approach to problem-solving
Experience: Group facilitation experience a plus; understanding of behavioral health and social service systems preferred
Work Environment & Benefits:
Location: Local travel required; work location determined by site assignment
Schedule: Hybrid work schedule available after 60 days; must work in-office three days per week
