The Case Manager (Mental Health Professional - MHP) supports the health and well-being of our clients through the provision of case management and counseling services. Essential Responsibilities

• Provide case management services
• Complete therapeutic assessments and work with each person to develop person-centered recovery goals
• Counsel clients, including crisis counseling and intervention
• May be required to run therapeutic skills groups
• Facilitate communication with client’s families, guardians, and parole and probation officers
• Facilitate linkage between client and community agencies
• Advocate for client rights and well-being.

Qualifications

• Bachelor’s or master’s degree in social work, psychology, or related discipline
• 1-3 years’ mental health experience preferred
• Hours may vary
