Job Title: Case Manager
Shift: 12HR EVE: 11A to 11:30P & Every other week 1 additional 8HR Shift 11A to 7:30P required for 4HRS of OT. 2+ years of Case Management experience required. Acute care experience is required. RN License required, nursys must be on profile. BLS required. Will be required to float to sister campuses as needed. DEGREE ACCREDITATION MUST BE ON PROFILE or MUST HAVE 5 YEARS OF EXPERIENCE. HWL prescreen interview, Manager final interview and offer.
Radius Rule 50 miles from facility to perm address. FLOATING REQUIRED, Travelers are first to float, possibly float to other sister campuses as well. ALL RTO must be on submission, HCF will not be approving RTO after submission. (Please load all Demographic information at time of submittal: Address, email, full DOB, full SS, RN license number.) We will NOT be able to confirm anyone without this demographic information loaded in the profile. Culture is VERY important here. Travelers are welcomed into the units and are expected to function as core staff does. Hard document deadline is Monday prior to start. No exceptions for pending docs.
