We're looking for a compassionate, resourceful Case Manager to support a 50-unit senior housing community in Buellton, CA.
This is a hands-on, on-site role where no two days look the same - and where your work directly impacts people's ability to remain safely and successfully housed.
What You'll Be Doing
Provide individualized case management based on each resident's needs
Coordinate community resources (mental health, financial assistance, senior services, veteran resources, etc.)
Collaborate with outside caseworkers, agencies, and community partners
Support residents transitioning into housing after homelessness
Help residents navigate challenges such as health needs, housing stability, and organization/hoarding concerns
Support and facilitate community activities like meetings, groups, or on-site programs (gardens, classes, wellness events)
What We're Looking For
Bachelor's degree (required) or Master's degree (MSW, MFT, or related, preferred)
A minimum of 3 years of case management or related social services experience (depending on education level)
Experience working with seniors, formerly unhoused individuals, or high-acuity populations is strongly preferred
Comfortable working independently while collaborating with external partners
Strong communication, organization, and follow?through skills
Spanish speaking is a plus, but not required
Monday-Friday, 8:30 AM-5:30 PM | 100% on-site in Buellton, CA
