Case Manager BSW

As a Case Manager BSW at Novant Health Huntersville Medical Center you will participate in the complex discharge planning process.  Huntersville Medical Center is an award winning 151 bed hospital in Huntersville, a family-friendly town in the Lake Norman area. 

This part-time position offers you more flexibility in your schedule while still offering part-time benefits.  The schedule will rotate with 3 8 hour shifts and one full weekend every 6 weeks.  Youmust be able to orient on a Monday - Friday basis for 4-8 weeks depending on experience.

• Education: Bachelor's Degree in Social Work from school accredited by the Council on Social Work Education is required.
• Experience: Two or more years professional social work experience or two or more years healthcare experience, preferred.  Case Management or related experience, preferred. 
• Additional Skills (required): Weekend and holiday coverage.  Must work one full weekend every 6 weeks.
• Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding.

• Participate in complex discharge planning process as a valued member of the healthcare team
• Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans
• Collaborate with RN Case Managers for guidance with medically complex cases
• Serve as a resource for discharge planning and care coordination for psychosocially complex cases, working with MSW Case Managers as needed
• Responsible for facilitating coordination of care
• Maintain current knowledge of available community resources and post acute care options
• Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health
