Case Manager BSW

What We Offer

As a Case Manager BSW at Novant Health Presbyterian Medical Center, you will serve to facilitate complex discharge planning for your assigned unit. You'll collaborate with RN and MSW Case Managers and work together with the physicians to plan and implement safe discharge plans. Presbyterian Medical Center is our 661 bed level 2 trauma center centrally located just outside of uptown Charlotte.

What You'll Do

• Participate in complex discharge planning process as a valued member of the healthcare team
• Collaborate with physicians and other members of the healthcare team to assess, plan, implement, and evaluate proactive, complex and safe discharge plans
• Collaborate with RN Case Managers for guidance with medically complex cases
• Serve as a resource for discharge planning and care coordination for psychosocially complex cases, working with MSW Case Managers as needed
• Responsible for facilitating coordination of care
• Maintain current knowledge of available community resources and post acute care options
• Accepts responsibility, authority and accountability for management of care in accordance with the current policies and procedures, mission, vision, and values of Novant Health

What We're Looking For

• Education: Bachelor's Degree in Social Work from school accredited by the Council on Social Work Education, required.
• Experience: Two or more years professional social work experience or two or more years healthcare experience, preferred. Case Management or related experience.
• Additional Skills (required): Weekend and holiday coverage.
• Additional Skills (preferred): Knowledge/Competence of Microsoft office products and keyboarding.
