Description

A nonprofit services organization in New York City is actively seeking a new Case Manager to join their growing team in Brooklyn, supporting residents through advocacy, assessment, service coordination, and client-centered care.

About the Opportunity:

  • Shift: Day
  • Schedule: Monday to Friday (with a Saturday rotation, one late day per week, and occasional summer Friday adjustments)
  • Hours: 9am to 5pm (with one late day per week until 6pm to 7pm)
  • Setting: In-person residential services

Responsibilities:

  • Manage a caseload of clients and provide regular individual case management support
  • Assess client needs and connect residents with appropriate health and human service providers
  • Escort clients to scheduled and unscheduled medical and psychiatric appointments as needed
  • Maintain accurate client records, including progress notes, service plans, assessments, and updates
  • Organize and support group activities, unit inspections, meal service, and follow-up on client needs

Qualifications:

  • At least 2 years of experience in Human Services and/or a related field
  • Bachelor's degree
  • Proficiency in Microsoft Office, email, and internet navigation
  • Strong spoken and written communication skills

  • Ability to manage multiple priorities in a fast-paced environment

Desired Skills:

  • Experience in Case Management
  • Knowledge of Homelessness, HIV, Substance Abuse, Mental Illness, and Entitlements
  • Skilled in Crisis Intervention
  • Familiarity with HIPAA and confidentiality protocols
