Case Manager

Coordinate with healthcare providers, social services, and other community resources to ensure comprehensive support for clients. The main responsibility of this role is to provide case management services for a caseload of approximately 22 clients.

• Conduct thorough assessments of clients'needs and develop personalized service plans.
• Maintain regular communication with clients to monitor progress and adjust plans as necessary.
• Coordinate with healthcare providers, social services, and other community resources to ensure comprehensive support for clients.
• Document all client interactions and maintain accurate records in compliance with organizational and legal standards.
• Participate in team meetings and contribute to the development of best practices in case management.
• Advocate for clients'needs and rights within various systems and institutions.
• Provide crisis intervention and support to clients as needed.
• Ensure confidentiality and privacy of client information at all times.
