Description

The Case Manager (Credible Messenger) provides intensive, trauma-informed case management and supportive services to young adults (ages 16–25) at elevated risk of engaging in serious violence. Grounded in the Cure Violence Global model, the Sanctuary Model, and Trauma-Informed Care, the Case Manager serves as the participant's primary point of support by building trusting relationships, coordinating services, providing crisis intervention, and promoting long-term stability. TITLE: Case Manager (CREDIBLE MESSENGER) – B.R.A.G. Restore

REPORTS TO: PROGRAM DIRECTOR, B.R.A.G. WEST/RESTORE

LOCATION: 1995 Jerome Ave., Bronx, NY & 1 East Fordham Rd. Bronx NY

HOURS: 35 Hours/Week, Non-Exempt

SALARY: $48,000 - $53,000

MAJOR DUTIES:

Participant Engagement & Case Management

  • Maintain a caseload of approximately 7–8 participants.
  • Complete trauma-informed intake assessments using the At-Risk Criteria (ARC) screening tool.
  • Develop and update individualized Sustainable Growth Plans based on participant strengths, needs, and goals.
  • Conduct weekly case management sessions and maintain consistent participant engagement through office, home, hospital, and community visits.
  • Build trusting relationships through mentorship, advocacy, and participant-centered support.
  • Engage families and natural supports when appropriate.
  • Monitor participant progress and adjust service plans as needed.

Crisis Intervention & Violence Prevention

  • Assess participant safety and develop individualized safety plans.
  • Respond to crises involving violence, victimization, housing instability, behavioral health, family conflict, or legal involvement.
  • Utilize trauma-informed interventions, motivational interviewing, restorative practices, and de-escalation strategies.
  • Collaborate with outreach staff during conflict mediation and community violence response efforts.

Service Coordination & Advocacy

  • Coordinate referrals and ongoing service connections for behavioral health, substance use treatment, housing, healthcare, legal services, food assistance, public benefits, domestic violence services, and family supports.
  • Advocate for participants with community agencies, healthcare providers, schools, courts, and other service systems.
  • Monitor service engagement and assist participants in overcoming barriers to accessing care.

Mentorship & Group Facilitation

  • Provide individual mentoring and supportive counseling that promotes healthy decision-making, emotional regulation, conflict resolution, accountability, resilience, and self-advocacy.
  • Plan, coordinate, and facilitate workshops, ensuring engaging and effective delivery of training and learning sessions.
  • Facilitate or co-facilitate psychoeducational groups focused on violence prevention, trauma recovery, restorative practices, emotional wellness, healthy relationships, stress management, and personal growth.
  • Encourage participation in program activities that support individual goals.

Documentation & Compliance

  • Maintain accurate and timely participant records, including assessments, case notes, progress notes, safety plans, referrals, and incident reports.
  • Enter participant information into Salesforce and other required databases within established timelines.
  • Maintain confidentiality and comply with agency policies and applicable regulations.
  • Review, complete, and approve participant timesheets to ensure accuracy, compliance, and timely payroll processing.
  • Manage and facilitate the participant onboarding process within ADP(Payroll System), ensuring accurate and timely completion of required documentation and system setup.

Team Collaboration

  • Participate in supervision, case conferences, staff meetings, training, and community meetings.
  • Collaborate with program staff while maintaining clear professional roles.
  • Support program events, quality improvement efforts, and other duties as assigned.

QUALIFICATIONS:

  • Required: High School Diploma/GED & a minimum of six years' experience providing case management, counseling, youth development, or related services.
  • Preferred: Bachelor’s degree in Social Work, Human Services, Psychology, Criminal Justice, Sociology, or a related field & a minimum of two years' experience providing case management, counseling, youth development, or related services.
  • Experience working with justice-involved, violence-impacted, or high-risk youth and young adults preferred.
  • Knowledge of trauma-informed care, crisis intervention, community resources, and case management practices.
  • Strong assessment, organizational, documentation, and communication skills.
  • Ability to establish effective relationships with participants and collaborate within a multidisciplinary team.
  • Proficiency with Microsoft Office and electronic case management systems.
  • Ability to travel throughout New York City and work flexible evening and weekend hours.
  • Credible Messenger with relevant lived experience is strongly preferred.
  • Experience with Motivational Interviewing and restorative practices.
  • Familiarity with the Bronx and surrounding communities.
  • Bilingual (English/Spanish) preferred.
  • Valid New York State Driver License preferred.
  • Ability to conduct frequent field-based work, including home visits and community outreach.
  • Ability to walk, stand, and travel throughout the Bronx for extended periods.
  • Ability to lift up to 25 pounds.
  • Ability to respond effectively during participant emergencies and crisis situations.
