Case Manager
A great community healthcare facility located near Boston, Massachusetts, is seeking a Case Manager to join their amazing team.
Pay Ranges from $40.00 - $53.66/hour
Case Manager Opportunity:
- This is a part-time, permanent, direct-hire opportunity
- Schedule: Day Shift
- Setting: Inpatient

Case Manager Responsibilities:
- You will arrange safe post-hospitalization care, rehabilitation facilities, or home health services to prevent readmissions
- You will advocate for client rights, navigate insurance pre-authorizations, and respond immediately to crises such as housing instability or abuse
- You will evaluate patient or client needs, review medical/social records, and create tailored treatment or care plans
Case Manager Qualifications:
- You must be qualified to independently complete an assessment within the scope of practice within your discipline (I.E. a Registered Nurse, Social Worker, Occupational Therapist, Physical Therapist, Speech Therapist, or Rehabilitation Counseling)
- You must have an active and unencumbered Massachusetts license of your discipline or a Compact License
- Must have or be eligible to obtain a CCM or ACM Certification
- You must have at least 2 years of Rehabilitation Experience
