    Raising Early Awareness and Creating Hope

    Starting Compensation:

    26.62 - 31.94 USD Per Hour

    Working Location:

    San Jose: North San Jose Center (In-Person Position)

    Working Hours/Shift:

    FT, Monday-Friday, AM shift (9 AM-6:30 PM)

    What you bring to SBHG (qualifications):

    Education/Experience:

    Applicants must meet any one of the following education and experience requirements:

    • HS degree with six (6) years of direct service experience in the Mental Health field, OR
    • Associates degree with four (4) years of direct service experience in the Mental Health field, OR
    • Bachelor's degree with two (2) years of experience working in the mental health field.

    License or Certification:

    Valid California Driver's License and current automobile insurance and must maintain a driving record that meets the company driver's eligibility policy.

    NOTE: This position requires driving from home to home, working with clients in the community, and transporting clients in personal vehicles.

    How you will make a difference (job overview):

    This position has primary responsibility for providing oversight and care management to the clients referred to Starlight programs. The Case Manager will provide services that may include case management/linkage, assessments, screenings, treatment planning/monitoring, and rehabilitation services to the clients and their families.

    Division/Program Overview:

    • Programs: Focus on providing prevention and early intervention to individuals (10 to 25 years old) who are at risk of experiencing early psychosis.

    • Population Served: (10 to 25 years old) who are at risk of experiencing early psychosis. We provide services to individuals and families throughout Santa Clara County.

    • Facility & Setting: Sessions are in the community, clinic, home-based and some telehealth.

    • Clinical Services: Focus on providing prevention and early intervention.

    • Skills & Services: Working as a team to provide quality and comprehensive care to individuals and their families.
