Case Manager - Bilingual (Spanish)

A nonprofit organization in New York City is seeking a new Case Manager for a promising opportunity to assist with community-based residential services in the Bronx.

About the Opportunity:

• Shift: Days

• Schedule: Sunday to Thursday

• Hours: 9am to 5pm (one day a week will be 10am to 6pm)

• Setting: Onsite

Responsibilities:

• Provide case management support to an assigned caseload of residents

• Assist clients with access to medical, behavioral health, substance use, vocational, food, clothing, and public benefit services

• Conduct follow-up, document services, and maintain accurate case notes in compliance with agency standards

• Accompany residents to appointments and support continuity of care through referrals and coordination with service providers

• Support daily living skills, community integration, and other resident needs in a residential program setting

• Perform other duties, as needed

Qualifications:

• 3+ years of experience working with a Behavioral Health population

• Bachelor's Degree

• Working knowledge of email, Microsoft Word, Excel, PowerPoint, and Windows operating systems

Desired Skills:

• Bilingual (English and Spanish)

• Experience with Substance Use, Mental Health, or Homelessness Services

• Experience in Group Work, Tenant Organization, or Community Outreach

• Experience working in a Residential or Supportive Housing setting
