    Position Summary:The Case Manager plays a critical role in connecting clients with resources and ensuring they receive comprehensive, trauma-informed support. This position involves assessing client needs, developing individualized care plans, coordinating services, and advocating for clients to achieve stability and wellness.

    Key Responsibilities:

    • Conduct thorough assessments to identify client needs and goals.
    • Develop and implement individualized service plans.
    • Coordinate referrals to community resources, healthcare providers, and social services.
    • Monitor client progress and adjust plans as needed.
    • Maintain accurate and timely documentation in compliance with agency and regulatory standards.
    • Collaborate with multidisciplinary teams to ensure holistic care.
    • Advocate for clients to access necessary services and overcome barriers.

    Qualifications:

    • Bachelor’s degree in Social Work, Psychology, or related field (Master’s preferred).
    • Experience in case management or social services strongly preferred.
    • Knowledge of community resources and trauma-informed practices.
    • Strong organizational, communication, and problem-solving skills.
    • Ability to work independently and as part of a team.
