TITLE:Adult Case Manager- Montgomery County Stepping Up Program

DEFINITION:
 
This position provides intensive case management services and screening for adults with Serious Mental Illness (SMI) and co-occurring mental illness and substance use disorders.   This position coordinates and provides services related to the “Stepping Up” program.  The Stepping Up Case manager’s duties include working closely with the State of Alabama “Stepping Up” Coordinator, the Technical Assistance Team and local community partners to implement processes and procedures to reduce recidivism and unnecessary re-entry of people with mental illness or co-occurring mental illness and substance use disorders into local county jails and hospitals.
 
DESCRIPTION OF DUTIES:
 
Provide intensive case management services to screen, assess, develop a case plan for and link consumers to appropriate mental health and social services.
 
Coordinate with Montgomery County stakeholders (jails and emergency rooms) to ensure that individuals are screened for mental illness and/or substance use disorders and referred to the Stepping UP case management program as appropriate.  
 
Participate in the local planning committee to support local policies and community outreach to mobilize community support.  
 
Complete a SUN Assessment Form on each client utilizing information gained from the client or a significant other.
 
Determine resources that would meet the needs identified in the SUN Assessment or from the crisis encounter by contacting provider sources, public and private, both in and out of the catchment area.
 
Develop an Individual Service Plan based on the needs assessment, and update as required and appropriate. 
 
Implement plan by providing case management services in conjunction with county jails and area hospital staff.
 
Assist in obtaining and coordinating social meaningful day services for the individual, including services relating to daily living activities, transportation services, habilitation and rehabilitation services, prevocational and vocational services, and housing services.
 
Assist the individual in obtaining income support services, including housing assistance, food stamps, and supplemental security income benefits.
 
Refer the individual for other appropriate services.
 
Complete reports required for client records as well as all forms and paperwork required by the Agency and the Stepping Up program.
 
Transport clients to services as needed to resolve crisis.
 
Track consumers discharged from hospitals, crisis residential facilities, or the diversion center. 
 
Participate in staffing on a regular basis with other treatment team members.
 
Participate in training activities as required to increase and enhance professional and therapeutic skills, including Stepping Up trainings. 
 
Conduct any “Stepping Up” related trainings, marketing and community outreach activities.  
 
Ensure consumers are transitioned to a lesser or greater restrictive level of care when appropriate.
 
Provide mental health consultation to other agencies or providers assisting the agency’s consumers.
 
Document all services within the Electronic Health Record.
 
REQUIREMENTS:
 
Knowledge of psychotropic drugs concerning their use, misuse, desirable and undesirable effects, and research materials available on these drugs.
 
Knowledge of assessing the mental status of individuals in need of mental health services.
 
Considerable knowledge of mental health services and available resources.
 
Ability to establish and maintain good working relationships with consumers, families, hospital staff and community mental health center staff.
 
Ability to work in a proactive, assertive, and energetic manner in fulfilling the duties of this position.
 
Willing to work in non-traditional and in-home settings.
 
Ability to work a flexible schedule, including evenings and/or weekends if necessary.
 
 
Must complete and receive certification in ADMH approved case management training upon hire.
 
Reliable transportation and willingness to transport clients.
 
Ability to work effectively as a treatment team member.
 
Ability to interact and communicate effectively with other professionals and other agencies.
 
Ability to organize and manage one's own activities with a minimum of direct supervision.
 
Ability to work flexible schedule including evenings and weekends if necessary.
 
Willing to provide on-call services if necessary.
 
Ability to quickly establish rapport with clients.
 
QUALIFICATIONS:
 
Bachelor's Degree in Behavioral Science or related field from a recognized college or university plus two years direct, related experience. 
 
Must meet requirements for Approved Medicaid Provider Status. 
 
Must hold a valid Driver's License and maintain a driving record that is acceptable to Carastar's insurance carrier.
 
 Must maintain at least liability coverage on personal vehicles.
