Position Summary

Desert Willow Wellness is seeking an experienced and compassionate Case Manager / Addiction Counselor to join our team in Palm Springs.

This role works directly with clients receiving detox and residential treatment for substance use disorders and co-occurring mental health challenges. The Case Manager manages an assigned caseload, provides individual and group counseling, supports treatment planning, and plays an important role in helping clients prepare for their next stage of recovery.

We are a small residential program, allowing our team to know our clients, collaborate closely across disciplines, and provide individualized support rather than working within a high-volume environment.

We are looking for someone who is clinically grounded, organized, dependable, and comfortable balancing meaningful client interaction with the documentation and coordination responsibilities that are essential to good case management.

Schedule

Wednesday - Friday: 12PM - 8PM 

Saturday: 10AM - 6PM 

What You'll Do

• Manage an assigned caseload and serve as a consistent point of support throughout each client's stay
• Meet individually with clients to address treatment goals, barriers, progress, and discharge needs
• Facilitate process, psychoeducational, life skills, and recovery-focused groups
• Complete required assessments, bio-psychosocials, treatment plans, progress documentation, and discharge summaries
• Develop and update individualized treatment plans in collaboration with clients and the clinical team
• Coordinate discharge and continuation-of-care planning, including referrals and connections to appropriate resources
• Collaborate closely with therapists, medical staff, BHTs, leadership, and other members of the treatment team
• Communicate with families and outside providers or agencies when appropriate and authorized
• Provide crisis intervention, advocacy, support, and de-escalation when needed
• Maintain accurate, timely, and compliant documentation
• Participate in clinical meetings, case conferences, supervision, trainings, and team collaboration
• Perform other related duties consistent with the scope of the position

We are looking for someone who can build rapport with clients while maintaining appropriate boundaries, communicate effectively with a multidisciplinary team, stay organized, and follow through.

Candidates should have:

• Strong knowledge of substance use disorders, recovery principles, co-occurring mental health conditions, and 12-step programs
• Experience providing individual and group counseling
• Strong case management, treatment planning, and discharge planning skills
• Crisis intervention and de-escalation skills
• Strong written communication and clinical documentation skills
• Ability to manage competing priorities and maintain required documentation timelines
• Ability to work independently while remaining collaborative and communicative with the treatment team
• Strong professional boundaries, judgment, and confidentiality
• Experience working effectively and respectfully with diverse populations, including LGBTQIA+ individuals

Minimum Qualifications

• Minimum of two years of experience working with individuals with substance use disorders and co-occurring mental health challenges
• Experience in behavioral health, addiction treatment, counseling, social work, or a related field
• CADC-I or CADC-II certification required
• Experience with electronic health records and clinical documentation systems
• Current CPR and First Aid certification
• Current TB test in compliance with facility requirements

Required facility-specific training, including IMS/medication training, UDS procedures, crisis intervention/de-escalation, suicide risk awareness, and other required training will be completed or verified as part of onboarding.

Work Environment

This position is based at our residential treatment facility in Palm Springs and involves regular interaction with clients in early recovery. Our team works collaboratively to maintain a structured, supportive, respectful, and clinically appropriate treatment environment.

Location: Palm Springs, CA, in person

Job Type: Full-Time or Part-Time

Schedule: Four days per week, Wednesday through Saturday

Pay: $24 – $27 per hour
