Description
Manage all new referrals for the SPIRIT outpatient program and begin their services. Work within a team approach.
Job Responsibilities
- Manage all new referrals for the SPIRIT outpatient program and begin their services.
- Work within a team approach to assist clients with overall coordination of basic needs such as Housing need; any financial needs such as social security or general assistance.
- Provide transportation to doctor appointments as needed.
- Conduct group sessions.
- Maintain program statistics.
Qualifications
- Bachelor's Degree (BA/BS) or BSW with some experience with community resources.
- Ability to be flexible with their work schedule.
- Valid driver’s license with an acceptable driving history.
