Description
Provide case management services to individuals residing in the Ozanam Inn Shelter.
Screening, assessment, service planning, linkage, crisis intervention, and de-escalation, managing relationships with a broad range of community providers, and life skills teaching.
Job Responsibilities
- Provide case management services to individuals residing in the Ozanam Inn Shelter.
- Screening, assessment, service planning, linkage, crisis intervention, and de-escalation, managing relationships with a broad range of community providers, and life skills teaching.
- Transporting individuals to housing opportunities, assisting client in applying for housing, building connection with other agencies and utilization of HMIS (WellSky) case management software and MyEvolv.
- Requires on-call coverage and evening hours.
- Bi-lingual (Spanish) encouraged to apply.
- Ability to work independently.
Qualifications
- Bachelor’s degree (B.A), preferably in Social Work or a similar Human Service field is preferred.
- Experience with conflict resolution and mediation.
- At least two (2) years relevant work experience and demonstrated ability to provide effective services to clients required.
- Must have valid driver’s license with acceptable driving history.
