    Schedule/Hours: Full-Time and One Weekend Day Per Week Required.

    This position is responsible for:

    Interviews, accepts, and provides comprehensive, long-term structured, complex, case management services for an assigned caseload of clients participating housing shelter program; provides coaching hours to promote an individualized housing plan is created and implemented by clients; understands the uniqueness of the client's history in order to determine most effective program plans; advocates for clients, and promotes a positive outcome. This position will service clients in a specialized Veterans program and a secondary emergency shelter program.

    Key Responsibilities:

    • Establishes goals with clients to promote stability and the securing of stable housing; communicates with clients in a manner that is easily understood; completes progress notes and modifies goals, as needed,
    • Refers clients to additional professional counseling resources, if needed; maintains awareness of progress with external case management professionals, working as a team to meet the holistic needs of the individual.
    • Meets regularly with clients to discuss and evaluate their progress; prepares accurate and up-to-date records documenting the same; completes incident reports, receives relevant incident reports and enforces policies and procedures, including but not limited to behavior and compliance plans.

    Physical Requirements and Working Conditions:

    • Ability to meet attendance requirements.
    • Ability to read, write, and communicate the English language effectively. Limited amount of physical effort required associated with walking, standing, lifting and carrying light objects (less than 25 lbs.) 5-10% of work time.
    • Duties are usually performed seated. Sitting may be relieved by brief or occasional periods of standing or walking.
