Our team is passionate, collaborative, and deeply committed to health equity and human dignity. What You’ll LeadAs Case Management Director, you’ll set the clinical and operational direction for our Case Management program—making sure services are consistent, high-quality, and grounded in trauma-informed, harm-reduction, whole-person care. Apply today. A Typical WeekdayYou’ll be onsite Monday–Friday, 8:00–5:00, starting with a quick pulse-check on program metrics and any overnight updates from outreach. Your day blends big-picture planning with real-time support: huddles with Case Management Supervisors, quick consults with multidisciplinary partners, and purposeful check-ins that keep communication smooth across clinic and community sites. Between meetings, you’ll review dashboards and documentation patterns, spot trends, and tee up practical improvements the team can actually use. Expect a steady rhythm of collaboration—problem-solving barriers to care, keeping workflows steady during change, and making sure the program stays mission-aligned and audit-ready.
