Reporting to the Manager of Case Management, the Case Manager Assistant supports the case management team in coordinating a smooth and timely discharge process. This role facilitates discharge services, appointments, DME needs, and access to community resources. The ideal candidate is organized, communicative, compassionate, and able to problem-solve effectively.
Job Responsibilities
- Coordinate post-discharge services, appointments, and required documentation.
- Collaborate with DME providers to ensure equipment is available at discharge.
- Communicate effectively with patients and families regarding discharge needs.
- Assist with connecting patients to community resources and support services.
- Support team conference documentation and compliance requirements.
- Maintain accurate and timely records.
- Promote a positive patient experience through compassionate communication and service.
Job Qualifications
- High school diploma or equivalent required.
- One year of healthcare experience preferred.
- Strong verbal and written communication skills.
- Computer proficiency.
- Excellent organizational and time-management skills.
- Strong problem-solving and follow-through skills.
