    Position Overview:

    Coordinates and implements the transition of care (TOC) / Discharge (DC) plan for ambulatory patients. Prioritizes and coordinates the plan across the care of continuum through critical thinking, teamwork, and communication between care providers, patients, and families.

    The Case Management Assistant is a member of the interdisciplinary complex care team and an extension of the patient physician team. Responsible for the management of patient information from internal and external sources to support the goal of providing patient-centered care to patients who have chronic diseases, complex medical conditions and who are in transition across different care settings. Coordinates care and provides telephone support for patients as a member of the interdisciplinary team following the plan of care, to achieve optimal results by impacting the delivery of quality care in a holistic approach to the patient and family. Schedules and maintains patient and provider appointments. Supports providers and other members of the health care team in clinic offices by performing a variety of back-office tasks and ensuring smooth patient and workflow within the department.

    Job Description:

    LOCATION & SCHEDULE:

    • This position is based primarily in Oakland, with occasional staffing at Castro Valley (based on operational needs). Monday through Friday, 8:00 AM to 4:30 PM.

    EDUCATION:

    • HS Diploma or General Education Diploma (GED)


    TYPICAL EXPERIENCE:

    • 1-year recent relevant experience.


    SKILLS AND KNOWLEDGE:

    • Oral and written communication skills.
    • Interpersonal and time management skills
    • Ability to work effectively in a fast-paced environment with rapidly shifting priorities and competing demands.
    • Ability to work independently with a minimum of direction.
    • Ability to exercise discretion and prioritize tasks, seeking input as indicated.
    • Intermediate PC skills and word processing skills required.

    Pay Range is $33.66 to $47.12 / hour

    The compensation range may vary based on the geographic location where the position is filled. Total compensation considers multiple factors, including, but not limited to a candidate’s experience, education, skills, licensure, certifications, departmental equity, training, and organizational needs. Base pay is only one component of Sutter Health’s comprehensive total rewards program. Eligible positions also include a comprehensive benefits package.
