Summary:
• We're looking for a compassionate and dedicated CASAC-T to join our 24-Hour Transitional Housing Program.
• In this role, you'll provide substance use counseling, recovery support, and care coordination to adults transitioning from homelessness, hospitalization, treatment programs, or other levels of care.
• As part of a multidisciplinary team, you'll help residents develop the skills, supports, and treatment connections needed to achieve long-term recovery, maintain stability, and successfully transition into permanent housing and independent living.
What You'll Do:
• Provide individual substance use counseling using a person-centered, recovery-oriented approach.
• Facilitate psychoeducational, relapse prevention, and recovery support groups.
• Complete substance use screenings, assessments, and individualized treatment plans.
• Conduct 90-day treatment plan reviews and monitor residents' progress toward recovery goals.
• Manage a caseload of residents with current or past substance use disorders.
• Collaborate with Case Managers, Social Workers, Clinical Supervisors, nursing staff, and community providers to coordinate comprehensive care.
• Assist residents with discharge planning by connecting them to outpatient treatment, detoxification, rehabilitation, medication-assisted treatment (MAT), mental health services, and community resources.
• Advocate for residents to ensure timely access to behavioral health, medical, housing, and supportive services.
• Respond appropriately to behavioral health and substance use crises using trauma-informed and de-escalation techniques.
• Maintain accurate, timely, and compliant documentation in the Foothold/AWARDS electronic health record.
• Participate in multidisciplinary team meetings, case conferences, and required trainings.
• Engage family members and natural supports, when appropriate, to strengthen each resident's recovery plan.
• Promote life skills, relapse prevention strategies, harm reduction, and healthy coping mechanisms that support long-term independence.
What We're Looking For:
Required Qualifications:
• 1 year of experience in a residential setting preferred
• CASAC-T credential required
• Strong interpersonal, counseling, and communication skills.
• Excellent organizational, documentation, and time management abilities.
• Ability to work both independently and collaboratively within a multidisciplinary team.
Preferred Qualifications:
• Experience working in transitional housing, residential treatment, supportive housing, behavioral health, homeless services, detoxification, rehabilitation, or community-based programs.
• Experience working with adults experiencing co-occurring mental health and substance use disorders.
• Knowledge of Motivational Interviewing, Harm Reduction, Trauma-Informed Care, Recovery-Oriented Care, and Crisis Intervention.
• Experience using electronic health records, preferably Foothold/AWARDS.
• Bilingual English/Spanish is a plus.
