Job Description - Care Transition Specialist (37782, 37783) (26141667)

Position Title:

Care Transition Specialist (37782, 37783) - (26141667)

To be considered for this position you must complete the state application and attach all required documents through the Montana State Careers site at www.statecareers.mt.gov.

The Montana Department of Public Health and Human Services (DPHHS) has a career opportunity within the Behavioral Health and Developmental Disabilities Division, Treatment Bureau.

The Behavioral Health and Developmental Disabilities Division (BHDD) aims to implement and improve a statewide system of prevention, treatment, care and rehabilitation for Montanans with mental disorders or addictions to drugs or alcohol. By teaming up with behavioral health specialists throughout Montana, BHDD provides substance abuse and adult mental health services for those in need of support.

The Care Transitions Section of BHDD will provide Critical Time Intervention (CTI) services to Montanans experiencing a major life transition, specifically low-income Montanans transitioning from Montana State Hospital to the community in the least restrictive setting.

CTI is a voluntary, phased, temporary intervention program that is evidence based and person centered. The goal of CTI is to assist with connecting individuals to necessary connections (professional and personal) and foster growth so that they have a successful transition from Montana State Hospital to the community, with the idea that they will leave the CTI program with the knowledge, tools, and supports in place to reduce future admissions.

This position is client-facing, and the work takes place in the community, not in an office setting. The person in this position will typically work business hours with frequent in-state travel, but the ability to be flexible will be required. The position will be responsible for working with clients and providers to make connections to care, as well as personal connections and fostering relationships. The position will report directly to the Care Transitions Program Supervisor.
