    Position Overview

    The Care Transition Coordinator is responsible for developing and managing referral relationships while positioning Aspire Cahaba as a provider of choice for rehabilitation and post-acute care.

    You'll serve as an important connection between referral sources, patients and families, the facility's admissions team, and facility leadership.

    You'll also monitor referral and census activity, identify opportunities for growth, and adjust outreach strategies to help achieve facility goals.

    What You'll Do

    • Develop and maintain strong relationships with hospital discharge planners, case managers, physicians, managed care organizations, and other referral partners
    • Generate and manage qualified inquiries and referrals
    • Conduct regular market visits and relationship-building activities
    • Educate referral partners about Aspire Cahaba's services and capabilities
    • Identify new referral opportunities and strengthen existing partnerships
    • Develop and execute measurable marketing and business development strategies
    • Monitor census and referral trends and adjust outreach efforts accordingly
    • Serve as a liaison between referral sources, facility leadership, and the Admissions and Marketing Coordinator
    • Coordinate the referral review and approval process according to company guidelines and protocols
    • Help coordinate the admission process from referral through arrival
    • Communicate with patients and families and assist with administrative questions related to admission
    • Maintain consistent follow-up with referral partners, patients, and responsible parties
    • Track outreach activity, referrals, admissions, and results
    • Represent Aspire Cahaba professionally throughout the healthcare community

    Qualifications

    Required

    • Experience in long-term care, post-acute care, managed care, or a related healthcare setting
    • Strong understanding of the healthcare referral and transition-of-care process
    • Demonstrated ability to build productive relationships with discharge planners, case managers, physicians, referral sources, patients, and families
    • Outgoing, energetic, and comfortable developing new professional relationships
    • Excellent verbal and written communication skills
    • Strong organizational and follow-up skills
    • Goal-oriented mindset with the ability to work independently in the market
    • Ability to manage multiple referrals and priorities in a fast-paced environment

    Preferred

    • Two (2) or more years of successful admissions, healthcare sales, referral development, or related experience
    • Community relations or healthcare business development experience
    • Established knowledge of the Birmingham-area healthcare market
    • Experience working with skilled nursing, rehabilitation, hospital discharge planning, or post-acute care
