Job Summary/Objective:

This position works primarily with the Youth Assessment Team.  This team will complete psychosocial assessments for the purpose of determining eligibility and ongoing care.  The goal of this team is to increase engagement into services, increase efficiencies with the assessment process provide additional support as customers access Summit Pointe’s services.

Essential Duties and Functions:

  • Completes psychosocial assessments inclusive of a diagnosis, case conceptualization, level of care assessment, and recommendations for treatment.
  • Engages in person-centered planning to develop goals and objectives. 
  • Utilizes evidenced based treatments within their scope of practice.
  • Engages in risk assessment and completes safety planning with youth and families.
  • Coordinates appropriate referrals to other youth-oriented programs such as Outpatient, Wraparound, and/or Youth Peer Support Services.
  • Performs outreach prevention activities within the community.
  • May conduct educational presentations.
  • Maintains record compliance according to agency guidelines and standards as well as funding source regulations. 
  • Shall ensure compliance and participation with competency improvement activities including, but not limited to credentialing, continuing education (per licensure requirements or a minimum of 24 hours per year of clinically based trainings), implementation of evidence-based practices, peer review attendance, and professional improvement practices.

Qualifications:

  • Bachelor or Master’s degree in human services related field.
  • One year of specialized training in treating or working with a minor and his/her family who live with a severe emotional disturbance preferred.
  • Licensure in the State of Michigan preferred.  Licenses may include LLBSW/LBSW, LLPC/LPC, TLLP/LLP, LLMSW/LMSW.
  • One year of experience in a behavioral health care setting utilizing evidence-based practices.
  • Child Mental Health Professional qualifications (as defined by the Michigan Department of Health and Human Services) is preferred.
  • Strong verbal and written communication skills.
  • One year of computer experience – Word, Excel and electronic medical record software preferred
  • Not excluded from participating in Medicaid, Medicare, or any other federal or Michigan health care programs.
  • Must support a trauma informed culture of safety to aid customers in their recovery process.
