Description

Job Summary/Objective: 

Care Managers are responsible for providing goal-oriented and individualized services and support, maintaining record compliance, participating in competency improvement activities, receiving training, and performing other duties as required.  

Essential Duties and Functions: 

  • Coordinate person centered plans for assigned customers; including arranging for meeting times and completion of the treatment plan and accompanying documents (i.e. crisis plans, advance directives, ASAM). 

  • Provide face to face treatment, as outlined within treatment plan, including core competencies of Case Management: assessment, planning, linking, coordinating, and monitoring. 

  • Provide services using resiliency, recovery focused, and evidence-based treatment. Services are provided in a trauma-informed process. 

  • Provide, coordinate and advocate for specialty populations (women’s specialty services, pregnant women). 

  • Coordinate and monitor community-based living arrangements (sober housing, supported independent living, etc.) for designated customers. 

  • Documentation is completed timely, strengths based, utilizes recovery methods and measures customer progress and satisfaction with services. 

  • Completion of SUD TEDS data at designated intervals, per SWMBH/MDHHS requirements. 

  • Complete ASAM assessments at designated intervals per SWMBH requirements. 

  • Act as liaison to detox, inpatient and/or residential facilities to promote appropriate treatment and discharge planning. 

  • Coordinate care with other external organizations as clinically necessary (treatment courts, MDHHS, courts, law enforcement, social security, transportation, community resources, medical providers, etc.), including jail diversionary activities as appropriate. 

  • Provide or arrange for crisis intervention services when needed, including inpatient referral. 

Qualifications: 

  • Bachelor’s degree in a human services field. 

  • One year specialized training or experience in working with substance use or co-occurring disorders. 

  • Two years of experience within a medical setting or behavioral healthcare setting preferred. 

  • One year of experience in working with youth experiencing a severe emotional disturbance preferred. 

  • One year of computer experience – Word, Excel, and other electronic medical record software preferred.
