Remote LMSW Care Manager Opportunity

We are seeking an experienced Licensed Master Social Worker (LMSW) for an excellent long-term temporary Care Manager opportunity with an established healthcare organization.

This position will transition to remote work after training; however, candidates MUST be local to the New York area and available to complete required training in the Bronx.

The Care Manager is responsible for coordinating comprehensive care for participants, reviewing assessments and documentation, collaborating with interdisciplinary teams, and ensuring members receive appropriate services based on their healthcare needs and goals.

Key Responsibilities

• Coordinate comprehensive care plans based on each participant's individual needs and goals

• Participate as an active member of the interdisciplinary care team

• Review assessments and documentation for quality and completeness

• Communicate regularly with field staff regarding participant care

• Provide coaching, education, and support to field staff as needed

• Coordinate community-based and long-term care services

• Collaborate with physicians, specialists, facilities, discharge planners, home care coordinators, and interdisciplinary team members

• Coordinate transitions of care between healthcare settings

• Monitor participants' responses to interventions and assist with modifications to care plans

• Coordinate service authorizations, assessments, reassessments, and ongoing services

• Maintain accurate and timely documentation in accordance with organizational, state, and federal requirements

• Conduct staff competencies and training as needed

• Support quality improvement initiatives and positive participant outcomes

REQUIRED Qualifications

• Active New York State LMSW license REQUIRED

• Master's Degree in Social Work (MSW) REQUIRED

• Minimum 2 years of relevant healthcare experience, preferably within managed care, CHHA, long-term home care, acute care, nursing home, or a related setting

• Experience working with frail and/or elderly populations

• Strong care management and care coordination experience

• Excellent communication, documentation, and organizational skills

• Ability to work effectively with interdisciplinary healthcare teams

• Managed long-term care experience is highly preferred

• Supervisory experience is a plus

• Bilingual skills are a plus
