Job Summary/Objective:
Care Managers play a crucial role in Medicaid, focusing on targeted case management and supports coordination. They assist customers in obtaining goal-oriented, individualized services and supports, including health, dental, financial assistance, housing, employment, education, and social services. They maintain record compliance, participate in competency improvement activities, and demonstrate good organizational, communication, and problem-solving skills.
Essential Duties and Functions:
Assists customers to design and implement strategies for obtaining services and supports that are goal-oriented and individualized.
Provides services such as assessments, planning, linkage, advocacy, coordination, and monitoring to assist customers in gaining access to needed services and natural supports developed through person-centered planning process.
Provides services in a responsive, coordinated, effective, and efficient manner focusing on process and outcomes.
Provides supports that may be provided within customers' homes or within community settings.
Maintains record compliance within agency requirements and standards utilizing electronic/hard copy record formats.
Qualifications:
Bachelor or Master’s degree in human services related field, required.
Licensure in the State of Michigan, preferred, LLBSW/LBSW. Other behavioral health licensures also accepted (LLPC/LPC, TLLP/LLP, LLMSW/LMSW).
One year specialized training or experience in treating or working with a person who lives with a Mental Health Diagnosis (QMHP) and/or with a person who lives with a Intellectual Developmental Disability (QIDP), required for those applicants who have a bachelor’s degree in a human services related field.
Strong organizational and verbal communication skills.
One year of computer experience – Word, Excel, and electronic medical record software preferred.
