The Internal Case Manager for Inpatient Rehabilitation, in collaboration with the multi-disciplinary team, coordinates the patient stay and discharge planning from the time of admission to the time of discharge. The Internal Case Manager supports the delivery of quality care in the most cost effective way by managing the human, material and financial resources available for the patient''s care needed for individual healthcare needs. The Internal Case Manager is also responsible for ensuring patient and patient''s caregiver(s) education in preparation for discharge readiness.

Posted are the minimum and the maximum wage rates on the wage range for this position. The successful candidate''s placement on the wage range for this position will be determined based upon relevant job experience and other applicable factors. These amounts are the base pay range; additional compensation may be available for this role, such as shift differentials, standby/on-call, overtime, premiums, extra shift incentives, or bonus opportunities.
