Salary Range: $23.19 – $24.60 per hour

Position Overview:

Kings View is seeking a Care Manager (Peer or Paraprofessional) to support Managed Care services. This role coordinates care, supports members, and promotes whole-person wellness.

Key Responsibilities:

• Outreach, engagement, assessments, and care planning

• Coordinate ECM and support services

• Connect members to housing, transportation, and benefits

• Advocate with providers and agencies

• Coordinate discharge planning and referrals

• Maintain timely documentation

• Collaborate with multidisciplinary teams

What You’ll Bring:

• High School Diploma or equivalent

• One year relevant experience

• Valid California Driver License

• Experience with homeless, SMI, SUD, or justice-involved populations

• BA/BS preferred; bilingual preferred
