Job Summary/Objective:
Care Managers are responsible for providing goal-oriented and individualized services and supports, maintaining record compliance, participating in competency improvement activities, receiving training, and performing other duties as required.
Essential Duties and Functions:
Provides Case Management services, such as assessments, planning, linkage, advocacy, coordination, and monitoring in an effective/efficient manner.
Connecting customers with appropriate resources and services.
Visiting customers within their homes and/or community settings.
Documents customer progress using both electronic and/or hard copy record formats.
Coordinates and operates under the guidance of a master’s level behavioral health clinician.
Actively supports culturally competent, recovery-base practices, person-centered planning as a shared decision-making process with the customer, who defines his/her life goals and is assisted in developing a unique path toward those goals.
Qualifications:
Bachelor’s degree in human services related field, required.
Licensure in the State of Michigan, preferred, LLBSW/LBSW. Other behavioral health licensures also accepted (LLPC/LPC, TLLP/LLP, LLMSW/LMSW).
One-year specialized training or experience in treating or working with a person who lives with a Mental Health Diagnosis (QMHP), required for those applicants who have a bachelor’s degree in a human service-related field.
