Care Management Support Coordinator II - Remote

Location: Houston, TX - Remote

Pay Rate: Starting at $21.00/hour

Schedule: Monday-Friday | 8:00 AM-5:00 PM

Dean's Professional Services is seeking a customer-focused and detail-oriented Care Management Support Coordinator II to support a leading healthcare organization in a fully remote role. This position provides administrative support for care management programs, including member and provider outreach, scheduling, documentation, resource coordination, and follow-up.

The ideal candidate is comfortable working independently, managing a high-volume workload, communicating with members and providers, and navigating multiple computer systems.

Responsibilities

  • Make daily outbound calls to members and healthcare providers to support care plan activities, scheduling, education, and follow-up.

     

  • Serve as a front-line contact for member and provider questions, requests, and concerns.

     

  • Manage a daily caseload of approximately 25-35 members.

     

  • Complete a minimum of 25 documentation notes per day based on established performance metrics.

     

  • Accurately document member interactions and activities in TruCare Cloud.

     

  • Review and log incomplete forms received from primary care providers using SharePoint.

     

  • Correct documentation errors and ensure member records are accurate and complete.

     

  • Assist members with connecting to health plan and community resources.

     

  • Identify and provide referrals for Social Determinants of Health (SDOH) needs.

     

  • Support member onboarding by sending welcome letters, correspondence, and educational materials.

     

  • Maintain accurate, non-clinical member records in accordance with state and regulatory requirements.

     

  • Communicate professionally and effectively with members, providers, and internal teams.

     

  • Perform other administrative and care management support duties as assigned.

     

  • Follow all applicable company policies, procedures, and quality standards.

     

Qualifications

  • High school diploma or GED required.

     

  • Previous experience in healthcare, care coordination, customer service, or an administrative support role preferred.

     

  • Must be comfortable working remotely and independently with minimal supervision.

     

  • Strong troubleshooting and problem-solving skills.

     

  • Computer proficiency with the ability to type at least 50 words per minute.

     

  • Proficiency with Microsoft Excel, Outlook, Teams, OneNote, and general computer applications.

     

  • Strong written and verbal communication skills.

     

  • Excellent attention to detail and documentation skills.

     

  • Ability to manage multiple tasks and meet daily productivity expectations.

     

  • Must have a personal cell phone for VPN login.

     

  • Ability to maintain confidentiality when handling sensitive member information.
