Care Management Support Coordinator II - Remote, Texas

Location: Remote - Texas | Hidalgo, Corpus Christi, and/or San Antonio Areas

Starting Rate:$21.00/hour

Schedule: Monday-Friday 8:00 AM-5:00 PM

Overtime: Available as needed

Dean's Professional Services is seeking a dependable and detail-oriented Care Management Support Coordinator II to support a leading healthcare organization remotely in Texas. This position is ideal for an experienced healthcare professional with strong communication, computer, and organizational skills who is comfortable working independently while communicating with members and providers.

Responsibilities

  • Make outbound calls to members and healthcare providers on a daily basis.
  • Document all member and provider interactions accurately in TruCare Cloud.
  • Review and log incomplete forms received from Primary Care Providers (PCPs) using the designated SharePoint site.
  • Identify and correct errors in documentation and notes as needed.
  • Complete a minimum of 25 notes per day in accordance with established productivity metrics.
  • Communicate professionally and effectively with members, providers, and internal teams.
  • Manage a daily caseload of approximately 25-35 members.
  • Maintain accurate and timely documentation while protecting member confidentiality.
  • Use Microsoft Office and other required technology to complete daily responsibilities.
  • Work independently while demonstrating strong troubleshooting and problem-solving skills.
  • Follow established healthcare, compliance, and organizational policies and procedures.

Qualifications

  • High School Diploma or GED required.
  • Minimum of 2 years of medical or healthcare experience required.
  • Previous experience in a call center, Primary Care Provider (PCP) office, managed care, or similar healthcare environment preferred.
  • Strong verbal and written communication skills.
  • Ability to work independently in a remote environment.
  • Strong troubleshooting and problem-solving skills.
  • Computer literate with a typing speed of at least 50 words per minute.
  • Proficiency with Excel, Outlook, Microsoft Teams, OneNote, and general computer applications.
  • Strong attention to detail and organizational skills.
  • Ability to manage multiple tasks while meeting productivity and documentation requirements.
  • Must have a personal cell phone capable of supporting VPN login.
